The HERACLES criteria of HER2 POSITIVE:

3+ HER2 IHC score in more than 50% of the tumor cells
OR
3+ HER2 IHC score in 10~50% of the tumor cells by IHC + FISH/DISH positive
OR
2+ HER2 IHC score in more than 50% of the tumor cells by IHC + FISH/DISH positive
